Yeah, this song is about Tobi Vail. Did some research, This is what i got.

"Apparently Kurt wrote this song about his old girlfriend Tobi Vail (Bikini Kill and Go Team). In an unsent letter addressed to Tobi he wrote:

"Every song on this record [In Utero] is not about you. No, I am not your boyfriend. No, I don't write songs about you, except for 'Lounge Act,' which I do not play, except when my wife is not around."

The song's about Tobi Vail and Calvin Johnson; "wear a shield" is a reference to K Records, which was founded by Johnson.

Always thought it was about weed ... lyrics seem to confirm my theory. Then again Cobain was known for making up random lyrics after music was written.
